1. An application addressed to the rector on the established form (the application form is issued by the Admissions Office)
2. Autobiography
3. Personal record sheet
4. Copy and original of identity card
5. A copy and original of the diploma of graduation from a medical university, or a certificate from the dean’s office
6. Copy and original of the diploma supplement
7. Health certificate (form 086/U) with all columns fully filled out
8. A copy and original of the military ID or registration certificate
9. 3×4 photographs (4 pcs.) cut out (photographs must be taken in the current calendar year)
10. The package of documents is submitted to the Admissions Committee of the Kazan State Medical Institute of Policy and Professional Development named after S.B. Daniyarov
